Dear Parents,
Consent for School Health and Dental Screening
The Health Promotion Board (HPB) will be conducting their annual health and dental screening in the school. Please read the attached letter from HPB for more information.
P1 parents: Complete health and dental consent via the consent portal (link below) to allow your child to receive any necessary vaccinations and undergo dental screening.
P5 parents: Complete health consent via the consent portal (link below) to allow your child to receive necessary vaccinations.
If you do not wish for your child to undergo HPB’s health screening in school, please write to ontact_YPS@hpb.gov.sg
